Psoriasis (PSO) Registry

To qualify for this study, you must:

1. Have been diagnosed with psoriasis by a dermatologist
2. Be at least 18 years of age or older
3. Be willing and able to provide written consent for participation in the registry
4. Be willing and able to provide Personally Identifiable Information (PII)that includes the following types of personal information at a minimum 1) Full Name and 2) Date of Birth.
   • Have started taking a new Enrollment Eligible Medication within 12 months prior to the Enrollment visit. A new medication is a medication that the subject has never taken before.
     OR
     Is prescribed a new Enrollment Eligible Medication at the Enrollment visit. A new medication is a medication that the subject has never taken before.
   • But for the enrollment of eligible medications, deucravacitinib and apremilast (generic and brand),the patient must be initiating (prescribed or starting) these treatments on the day of the enrollment visit.

Additional criteria will apply, which you can discuss with the study provider/staff.

If you decide to be in this registry, you will be asked to complete CorEvitas Registry Questionnaires during regularly scheduled visits every 6 months.

The registry does not have a dedicated stop time, however, you may leave the registry at any time. Your decision will not result in any penalty or loss of benefits.

By being in this study, you could help contribute to research. The information gathered from this registry may help people with psoriasis in the future. Reimbursement may also be provided.

There are no anticipated physical risks.

Yes, if you join the study, you may leave the study at any time and for any reason.
